Autofluorescence Endoscopy Laboratory (AFI)
SERVICE DESCRIPTION:
A complex system of endoscopic autofluorescence was inaugarated in Research Center of Gastroenterology and Hepatology Craiova in 2008, for the first time in Romania. Endoscopic autofluorescence is a new screening method for malignant lesions of digestive tract (severe dysplasia, early esophageal and gastric cancer, early colo-rectal cancer). The principle of autofluorescence diagnosis is based on the interaction between light at a specific wavelength and fluorophores in tissue. The detection of abnormal lesions with autofluorescence depends on changes in the concentration or depth distribution of the endogenous fluorophores, changes in the tissue microarchitecture, or both, including altered mucosal thickness or blood concentration, that can affect the fluorescence intensity. Autofluorescence endoscopy laboratory offer high performance diagnostics and therapeutic procedures: trimodal upper gastrointestinal tract endoscopic examinations: magnifying endoscopy with white light, autofluorescence endoscopy, narrow band imaging mode, as well as complex therapeutic procedures: polypectomy and endoscopic mucosal resections endoscopic band ligation of esophageal varices and haemostasis in gastrointestimal bleeding. All these procedures are completed by biopsies with complex pathological and immunohistochemical analyses.

Confocal Laser Endomicroscopy Laboratory (CLE)
SERVICE DESCRIPTION:
Confocal Laser Endomicroscopy (CLE) will play an important diagnostic role during gastrointestinal endoscopy in the future. The great advantage of CLE is the elimination of the diagnostic delay associated with conventional biopsy preparation and processing, enabling in vivo microscopic assessment of the tissue during examination. The technique is available from 2008 in Research Center of Gastroenterology and Hepatology Craiova. CLE has been recently proposed as a new technique that allows in vivo histological assessment of mucosa during ongoing endoscopy. The technique permits simultaneous endoscopic and endomicroscopic examination in real time. This allows imaging of the mucosal layer, including epithelial cells and the lamina propria. The targets of endomicroscopic examination for an accurate diagnosis can be the cells, vascular structures, and tissue patterns. The principle of CLE is based on the integration of a confocal laser microscope into the conventional endoscope permitting simultaneous endoscopic and endomicroscopic examination in real time. During procedure, a laser device delivers an excitation wavelength of 488 nm that is focused onto a distinct imaging plane within the tissue. The range of depth is 0-250 mm below the surface layer. Potentially appropriate contrast agents are represented by fluorescein, acriflavine, tetracycline, or cresyl violet. Clonfocal laser endomicroscopy has several important clinical applications: The diagosis of Barrett esophagus and early esophageal cancer In situ diagnosis of H. Pylori, premalignant and malignant gastric lesions The diagnosis of celiac disease The endoscopic surveillance of ulcerative colitis and Crohns disease Early stage diagnosis of colorectal cancer As a new hope for early detection of high-grade dysplasia, the combination of targeted peptide probes and visualization using CLE (“immunoendoscopy”) offers an appropriate method for facilitating in vivo early cancer detection.

Endoscopic Ultrasound Laboratory (EUS)
SERVICE DESCRIPTION:
The activity of Endoscopic Ultrasound Laboratory (EUS) started in 2001 using a high resolution digital linear ultrasound endoscope. In the Research Center of Gastroenterology and Hepatology Craiova many premiers were registered. Thus, over the years were used EUS elastogpraphy, second generation contrast enhanced EUS, EUS guided pancreatic pseudocysts drainage, EUS guided celiac plexus neurolysis, etc. Many of the procedures were registered in a database and the illustrative cases were published in Linear Endoscopic Ultrasound Atlas (www.EUSAtlas.ro), an open website hosted in web-servers of University of Medicine and Pharmacy Craiova. High performance procedures are currently performed: Linear endoscopic ultrasound procedures: color Doppler, contrast enhanced or elastographic EUS-Fine needle aspiration or biopsies in suspected tumor masses (tumors, lymph nodes, etc.), as well as EUS-guided therapeutic procedures: pseudocysts drainage, celiac plexus neurolysis, etc). All these procedures are completed with cytological or immunocytological exam of the samples in the presence of an experienced cytopathologist. Research samples (for immunohistochemical and molecular DNA or RNA studies) are stored and biobanked in collaboration with the Human Genomics Laboratory.
